Insurance

Dr. Sim does not accept any insurance at this time.  If you have private health insurance, she can assist in providing a bill to submit to your insurance company for any possible out-of-network benefits. However, you are responsible for all charges and checking with your health insurance company regarding any possible out-of-network benefits.  Dr. Sim is also not a Medicare enrolled provider. Therefore, you must enter into a private contract with Dr. Sim to obtain neuropsychological assessment services and agree not to submit a claim to Medicare or ask that she submit a claim to Medicare. 

Payment

Cash, check, VISA, MasterCard, Discover, and American Express are all accepted forms of payment.  Patients can also pay by debit, HSA, or FSA cards through Ivy Pay.  A deposit is required at the time of scheduling, with full remittance of the balance on the day of the appointment. 

Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. By law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. 

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

  • You have the right to receive a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate.
